What Bangkok sells is something different, and it is worth understanding before you spend anything. In Thailand, the surgeon who consults with you is the one who operates, the price does not climb when you ask for the senior doctor, and the aftercare runs for a year rather than a weekend. This guide gives you the 2026 prices from published Thai clinic price lists, the same figures for Istanbul, London and the United States so the comparison is real, how the graft market actually works, how many days you need, what recovery genuinely looks like month by month, and the specific checks that separate a surgeon from a production line.

All figures in US dollars. Most Bangkok clinics set a floor of 1,000 grafts, and charge a slightly higher rate with a minimum fee of around 59,000 baht for anything smaller. Sessions above roughly 3,500 grafts are usually split across two days or two trips, because there is a limit to how long a scalp tolerates being open.

How to read a per-graft quote without getting caught:
  • Ask what a graft means at that clinic. A graft is a natural grouping of one to four hairs, so 2,000 grafts might be 3,500 hairs or 5,500 hairs depending on your follicle grouping. Clinics that quote per hair rather than per graft are producing a number that looks lower and is not.
  • Ask for the graft count in writing before the day of surgery, not after. A quote that becomes a different number once you are in the chair is the oldest problem in this industry.
  • Ask what happens if the surgeon opens the donor area and finds less than expected. The honest answer is that they place fewer grafts and charge you for fewer.
  • Ask whether the price includes the first wash, the medication, and the follow-ups, and for how long. In Bangkok the answer is usually yes and twelve months, which is a large part of what you are paying for.

The number that should worry you is a low one

In every other kind of medical travel, a suspiciously cheap quote usually means an older facility or a less convenient city. In hair restoration it usually means something specific: that the person holding the punch is not a doctor. That is the subject of the next section, and it is the only part of this guide that genuinely changes outcomes.


🔍 Who Is Actually Holding the Punch

This is the question the entire industry is organised around avoiding, and it is the one thing you should decide before you compare a single price.

A hair transplant has three surgical steps. Somebody has to extract each follicular unit from the back of your head, somebody has to make the thousands of tiny incisions that decide the angle, direction and density of the new hairline, and somebody has to place a graft into each of those incisions. The extraction and the placement are technical and repetitive. The incisions are the part that decides whether you look like you had good genes or like you had surgery.

In much of the world, all three steps are performed or directly supervised by a licensed doctor. In the high-volume package markets, they are frequently not. The International Society of Hair Restoration Surgery, the field's main professional body, runs a standing campaign about exactly this, warning patients about unlicensed technicians performing substantial parts of hair restoration surgery, and about clinics operating with minimal or no physician involvement. Their concern is not snobbery about credentials. It is that a technician cannot diagnose the hair disorder underneath your hair loss, cannot tell you that you are a poor candidate, and has no incentive to.

Read the package tiers, they will tell you themselves

The clearest evidence available is not an expose, it is published pricing. Major Istanbul clinics list packages where the entry tier states plainly that the incisions and the implantation are performed by surgical technicians, the middle tier buys you a named doctor for the incisions, and the top tier buys the founding surgeons. The graft count and the technique are identical across all three. What changes is whose hands are on you. If a clinic sells doctor involvement as an upgrade, then the base price is telling you what you get without it.

Ask the question directly and in writing

Send one message before you pay a deposit anywhere: which named doctor performs the extraction, which named doctor makes the recipient incisions, and how many other patients is that doctor operating on the same day. A clinic that answers all three plainly is a clinic you can work with. A clinic that answers with the words team, expert staff or specialists has answered you.

Verify the doctor, not the clinic

Clinic names, awards and gold-lettered lobbies are marketing. Individual credentials are checkable. Every doctor practising in Thailand holds a Medical Council of Thailand registration. Beyond that, the two credentials specific to this field are a diplomate certification from the American Board of Hair Restoration Surgery and fellowship of the International Society of Hair Restoration Surgery, both of which name individual physicians rather than businesses, and both of which have public directories.

Count how many patients are booked that day

The economics of the mill model depend on a single doctor nominally covering several simultaneous operating rooms. A surgeon who takes one or at most two cases a day is telling you where the time goes. This is the question that most reliably separates the two business models, and it is the one clinics least expect to be asked.

Be suspicious of a big graft number offered early

Your donor area is finite and it does not grow back. The number of grafts you can safely harvest in a lifetime is set by your own scalp, not by your budget, and a surgeon who plans for the man you will be at fifty will hold some in reserve. A clinic that quotes 5,000 grafts before examining your donor density under magnification is selling volume. Even the large Istanbul clinics say this one out loud on their own pricing pages: the graft count follows what the donor can supply, not what you are willing to pay.

Almost everything else in cosmetic medicine is repairable, reversible or temporary. A badly harvested donor area is not. Over-harvesting leaves permanent thinning at the back and sides, which is worse than the problem you came in with, and it removes the option of ever fixing the front properly. Corrective work exists, it is charged at a premium, and it can only ever be partial. This is the reason to be slow about choosing.

A strip of skin is removed from the back of the head, closed with sutures, and dissected into grafts under microscopes. It leaves a fine linear scar, so it suits people who keep their hair longer than a number three. It is the cheapest per graft and it can yield a very high number of grafts in one sitting, which is why it still has a place for large cases and for people with limited donor density. Your existing hair is left long, and the grafts are placed between it, so nobody has to know. The same techniques applied to smaller areas. Eyebrows typically need 200 to 400 grafts per side and are priced per graft at the higher end because the angles are unforgiving. Beard transplants often use donor hair from the scalp. Not all hair loss is pattern baldness. The laser caps and helmets sit at the harmless-and-mildly-useful end of the spectrum. Several Bangkok clinics include unlimited use of one for twelve months inside the surgical price, which is a reasonable thing to accept and an unreasonable thing to buy at full retail.

If you are in the early stages, the sequence that saves the most money and the most hair is boring and works: see a dermatologist, get a diagnosis, start medication, hold for twelve months, photograph the same three angles under the same light every month, and only then decide whether you need surgery at all. Bangkok is a good place to do that first step, because a dermatology consultation at a private hospital here costs a fraction of what it does in most Western capitals and you can have the bloodwork back the same day.


📅 How Many Days You Actually Need

Clinics will tell you three days. That is true in the sense that you can technically leave, and misleading in every other sense. Five to seven is the trip that goes well.

Day one, arrival and consultation

Land, sleep, and see the doctor in person. A good clinic examines the donor area under magnification, agrees the hairline with a marker pen and a mirror while you are sitting up, and confirms the graft count. This is the last comfortable moment to change your mind, and you should treat it as a real appointment rather than a formality. Bloodwork is usually taken the same day.

Day two, surgery

Six to nine hours in the chair for a typical case, under local anaesthetic, awake, with breaks and lunch. The anaesthetic injections are the uncomfortable part and they last a few minutes. Most of the day is undramatic and slightly boring, which is the correct outcome. You walk out, you go back to the hotel, and you sleep propped up.

Day three, first check and the swelling

Back to the clinic for a check. Forehead swelling typically appears now and peaks around day three or four. It can be dramatic, it looks worse than it is, and it resolves entirely. Sleeping at forty-five degrees and a cold compress on the forehead but never on the grafts is the whole management plan.

Days four and five, the first wash

The clinic performs the first wash and teaches you the technique for doing it yourself. This matters more than it sounds: washing wrongly in the first two weeks is the most common way patients damage their own result. Get it done at the clinic at least once, and film it on your phone.

Day five onwards, you are free within limits

Grafts anchor progressively over the first ten to fourteen days as new blood vessels grow into them, and the highest risk window for dislodging one is the first forty-eight to seventy-two hours. By day five you can move around normally, eat out, sit in air conditioning and go to indoor attractions. What you cannot do is sweat, swim, sunbathe or let anything press on your head.

Flying is physically safe from about twenty-four to forty-eight hours, and most surgeons still prefer you stay longer. The argument for day five or later is not the flight itself, it is that you have had the first wash supervised, the swelling has peaked and passed, and the person who operated on you has looked at the result once more before you are eleven time zones away. Book a window seat and do not sleep against it.

You will make the trip between the two three or four times in a week, twice in the first three days, and Bangkok traffic turns a four kilometre journey into fifty minutes with no warning. A slightly worse hotel five minutes from your clinic is a better trip than a slightly better hotel across the river. Ask the clinic which hotels their patients use, since most have a short list and a negotiated rate.

The single most important thing to understand before surgery is that you will look worse before you look better, and that the worst point comes about a month after you fly home. People who do not know this in advance panic. People who do, do not.

Grafts sit in their channels without a blood supply of their own and without adhesion. Sleep on your back at forty-five degrees, do not touch the recipient area, do not wash your own hair, do not bend over, no alcohol and no smoking. This is the only period where a careless moment can cost you grafts, and it is short.

Days 4 to 10, washing and crusts

Gentle washing begins under instruction, and the crusts around each graft soften and start to come away. Walking is encouraged, everything more strenuous is not. By day ten the grafts are anchoring properly and the anxiety level drops sharply.

Crusting is usually finished by the end of week two, and normal showering with a direct stream is generally cleared around then, still without pressure or rubbing. The transplanted hairs fall out. All of them, usually. Roughly half to two thirds of the final density is visible. This is the point where most people stop worrying, and where good clinics schedule a follow-up and a photograph. It is not the final result and you should not judge the surgeon here.

  • Touching the grafts: not at all for 10 days
  • Sweating and the gym: nothing strenuous for 2 weeks, moderate training from week 3 to 4, heavy lifting and running from about week 4, contact sport from week 6 to 8
  • Swimming, pool or sea: nothing for the first month, and only once the scabs have fully cleared
  • Direct sun on the scalp: avoid entirely for the first weeks and treat roughly three months as the guide for unprotected exposure, which in Bangkok is the restriction that shapes your whole trip
  • Hats and helmets: nothing that presses or rubs for about 10 days, and the loose clinic-issued cap only
  • Alcohol and smoking: both restrict blood flow to healing grafts, so avoid for at least the first week and preferably longer

🗺️ What You Can Actually Do While You Heal

This is the part that makes Bangkok worth choosing over a two-night package trip, and it is also the part most people get wrong by booking the exact things they must not do.

A hair transplant leaves you fully mobile. You can walk, eat, sit, travel and hold a conversation from the day after surgery. The restrictions are narrow and specific: no direct sun on the scalp, no sweating, no swimming, nothing pressing on your head, and no exertion. In Bangkok in September that rules out almost everything outdoors between ten in the morning and five in the afternoon, and it rules in the entire air-conditioned half of the city, which is considerable.

The four things below are what we would send a patient to from around day three, in the order the week usually goes. Every one of them is indoor or after dark, seated or flat, short, and cool.

The gentlest thing on this list and the right first outing. It is entirely indoors, entirely flat, air-conditioned throughout, takes about an hour, and sits in the basement of Siam Paragon directly on the BTS, so you can get there and back without standing in the sun for a single minute. Low light, no crowds on a weekday morning, and nothing that will bump your head.

Same part of town, same logic, a different hour to fill. Fully indoor, cool, and the entire visit is walking slowly around a well-lit room, which is precisely the level of activity you are cleared for in the first week. Good for day four or five when you are restless but not yet allowed to do anything that raises your heart rate.

A lift straight to an indoor observation floor with the whole city underneath it, and the best view in Bangkok from a seated position. Go in the last hour before sunset. There is an open-air rooftop as well, and while it is the famous part, a freshly transplanted scalp has no business in direct sun, so stay behind the glass until the light goes.

What not to book for the first two weeks, however tempting:
  • Water parks and anything with a pool. Chlorinated water on healing grafts is the clearest no on this list, and it applies for a full month.
  • Open-air zoos and safari parks. Several hours in direct sun with no shade is the worst possible environment for a new scalp, regardless of how good the day out is otherwise.
  • Outdoor theme parks. Sun, heat, sweating and rides that jolt your head. Every single factor is against you.
  • Full-day temple tours upcountry. Ayutthaya is a wonderful day and it is nine hours of open sun and walking. Move it to the end of the trip or the next one.
  • Elephant parks and anything agricultural. Dust, heat, physical activity and mud, in a place where you cannot wash properly afterwards.
  • Islands and beaches. Sea water, sun and sand. This is the one people book before they read the aftercare sheet, and it is the trip to take twelve months from now when you have something to show off.

Against a London quote of 5,700 to 9,500 US dollars for surgery alone, Bangkok breaks roughly even and you get a week in Thailand out of it. Against a United States quote of 10,000 to 25,000, Bangkok is between a third and two thirds cheaper even after the flights. Against an Istanbul package at 2,600 to 3,150, Bangkok is roughly three times the price, and the entire question is whether the difference between a technician and a surgeon is worth three thousand dollars to you on a procedure you cannot undo.

That is your decision to make and not ours to make for you. What we would say is this: the people who regret a hair transplant almost never regret the amount they spent. They regret the hairline that was drawn too low and too straight for a man who is going to be sixty one day, the donor area that was harvested too aggressively, or the second surgery they needed to repair the first. Every one of those is a decision made by whoever planned the operation, and none of them are made cheaper by having flown further.

Count the week, not just the surgery

Charging the entire flight and hotel against the procedure is the harshest possible way to read the numbers, and it is not how the week actually goes. You are having surgery on one day out of seven. The other six you are a person in Bangkok with almost nothing wrong with them, eating well, sitting in the cool and seeing the city in the evenings. If you were ever going to take a holiday this year, this one is already paid for.


FAQ: Hair Transplants in Bangkok

Is Turkey not just cheaper and the same?

Cheaper, yes, by a wide margin, and there are excellent surgeons in Istanbul. The same, no. The difference is structural rather than national: the Turkish market is built on fixed all-inclusive packages at high volume, and doctor involvement in the surgical steps is frequently an upgrade you pay extra for, which the clinics themselves publish on their pricing pages. The Thai market prices per graft with a doctor performing as standard. If you go to Turkey, buy the tier where a named surgeon does the incisions, and the price gap closes considerably.

How many grafts do I need?

Nobody can tell you from a photograph, and anyone who does is guessing at your expense. As a rough frame, a receding hairline typically needs 1,500 to 2,500 grafts, a hairline plus mid-scalp 2,500 to 3,500, and a crown another 1,500 to 2,500 on top. The real number comes from measuring your donor density under magnification and dividing the supply between the areas that matter most, now and in twenty years.

Will people be able to tell?

In the first two weeks, if you had a shaven procedure, yes. After that it looks like a short haircut. The thing that makes a transplant obvious years later is not the surgery, it is the design: a hairline placed too low, too straight, or without the irregular soft edge a real hairline has. That is decided in fifteen minutes with a marker pen, and it is the strongest argument for paying for a surgeon rather than a technique.

Do I have to shave my head?

For standard FUE, the donor area is shaved and usually the recipient area too. Non-shaven and long-hair FUE avoid this entirely and cost roughly fifty percent more per graft, with a lower maximum graft count. If your work or your circumstances mean you cannot show up with a shaved head, this is a good use of money rather than a vanity upgrade.

Does it hurt?

The anaesthetic injections at the start are genuinely uncomfortable for a few minutes. After that you feel pressure and nothing sharp, and most people are bored rather than distressed by hour four. The first two or three nights are the sore part, managed with ordinary painkillers, and the donor area is tender for about a week.

Do I have to take finasteride forever?

Transplanted hair is permanent and is not dependent on medication. Your remaining native hair is, and it will keep receding without it. Some people accept that and plan a second procedure later, which is a legitimate choice. What is not legitimate is a clinic that never raises the question, because the plan they are selling you quietly assumes an answer.

What if the result is bad?

Ask before you book, in writing, and read the answer carefully. Several Bangkok clinics offer a free touch-up if density disappoints at the twelve-month assessment, which is meaningful because it aligns them with your outcome. What no guarantee covers is a badly designed hairline or an over-harvested donor area, because those are not density problems and cannot be topped up. Prevention is the only remedy there.

Can I go to the beach afterwards?

Not on this trip. Sea water and direct sun are both out for the first month, and unprotected sun on the scalp is best avoided for roughly three months. Plan Phuket or Krabi for the trip after, when you will have a result you want photographed. For this week, keep it indoor, keep it evening, and keep it cool.

Is it safe to combine with other treatment?

Dental work, a health checkup and dermatology all combine fine and are commonly done in the same week, usually before the surgery day rather than after. What does not combine well is anything requiring general anaesthetic, anything that puts you face-down, or anything that involves lying on the back of your head. Tell every doctor involved what else you are booked for.

When is the best time of year to come?

November to February, comfortably. The sun restriction and the sweating restriction are both far easier to observe in the cool season, and a scalp that must stay dry is a difficult thing to manage in the April heat or in a September downpour. If you can choose your month, choose one of those four.
